If you are a new patient to our office, please contact our office for new patient forms. 

Payment Options

For your convenience, our Banff, AB dental office accepts Visa, Mastercard, Debit Card, Personal Cheques and Cash. Should you be covered by a Dental Insurance Plan, in most cases, our billing computer software will process and complete your claim form. Full payment for any treatment is expected as service is delivered. Special financial arrangements may be made as needs require. Should you have any questions in regards to your Plan Coverage or payment obligations, our knowledgeable business staff is always willing to discuss your personal needs.
